To enroll your child(ren), the following documentation is required:

-A certificate of immunization (HRS Form 680)
this form is available from either a private physician or
the County Health Department

-Proof of a medical exam within the last 12 months.

-Kindergarten and first grade students must present proof of age.
A certified copy of a birth certificate, baptismal
certificate or passport is acceptable. For students
in grades 2-12 prior school records are accepted.

-Two documents for address verification
i.e. electric bill, rental agreement, telephone bill or
voter
